Crown Care didn't begin in a boardroom. It started with a phone call nobody wanted to make — to a Medicaid broker who had, for the third time in a month, sent a dispatcher who claimed they'd "never received" the standing dialysis reservation. The patient, in this story, was somebody's mother. The family decided that day that private pay medical transportation — built properly, staffed professionally, and held to a concierge standard — was long overdue in America.

The origin

Three problems with traditional NEMT, in three sentences.

First: Medicaid broker models create adversarial incentives — brokers profit on margin, not on service, so the vehicles and drivers they assign tend to be whoever will accept the lowest rate. Second: patients who are already physically vulnerable are the worst-positioned to advocate for themselves when a ride arrives 90 minutes late, wrong vehicle type, or doesn't arrive at all. Third: even people who can afford to pay out of pocket for better service often can't find better service — because the private pay NEMT market, until recently, barely existed.

Crown Care is our answer. We're a transportation company the way a five-star hotel is a lodging company: the core service matters, but the operating standards matter more.
